Getting started at KFTHC

In order to become a participant at KFTHC you will need to do some paperwork.

All riders need a statement from their doctor saying that therapeutic riding will not harm them.

Additionally, PATH, Intl. provides a list of precautions and contraindications KFTHC follows to ensure safety for its participants.

Below are the forms you will need to fill out, including a Physician's Statement Form for your convenience.  (If you have difficulty printing, call or email us and we can mail you the forms you need.)

Also below is a printable copy of the KFTHC Participant Manual which, we hope can answer many of the questions you may have.

You can also contact us at 606-965-2158 or email us at kfthc@prtcnet.org.

We are looking forward to meeting you!!
